What actually happens in your body when you have diabetes

If you've just been diagnosed with diabetes, or someone close to you has, you've probably been handed a lot of instructions and not much explanation. Check this. Avoid that. Take these. But almost nobody sits you down and tells you what's actually going on inside you, and why any of it is happening.

That's what this is for. No jargon you have to decode, no talking down to you. Just a clear picture of what your body is doing, so the rest of it starts to make sense.

Here's the whole thing in one sentence: diabetes means there's too much sugar in your blood, because that sugar can't get into your cells where it's supposed to be used for energy. Everything below is just the story of how that happens, and why it leaves you feeling the way it does.

First, the sugar isn't the enemy

When people hear "high blood sugar," they tend to picture sugar as something bad that's snuck into the body. But the sugar in your blood, called glucose, is meant to be there. It's your body's main fuel. Nearly everything you eat, whether it's bread, fruit, rice, or a birthday cake, gets broken down into glucose, and that glucose travels around in your bloodstream so it can reach every part of you that needs energy.

And everything needs energy. Your muscles need it to move. Your brain runs on it constantly. Even sitting still and reading this, your cells are quietly burning glucose to keep you going.

So glucose in the blood is normal and necessary. The problem in diabetes isn't that the glucose exists. It's that it gets stuck in the blood instead of getting where it needs to go.

The thing that lets sugar in: insulin

Glucose can't just drift into your cells on its own. It needs to be let in, and the thing that lets it in is a hormone called insulin.

A hormone is simply a chemical messenger your body makes to tell other parts what to do. Insulin's message is short and specific: it tells your cells to open up and take glucose in from the blood. When insulin is present, your cells absorb glucose, your blood sugar comes back down to normal, and that glucose gets put to use as energy. No insulin, no entry.

Insulin is made in an organ called the pancreas, which sits just behind your stomach. You never think about it, but every time you eat, your pancreas is paying attention.

What happens in type 1 diabetes

Type 1 is the more clear-cut of the two, which is why it's easier to start here.

In type 1 diabetes, the pancreas can't make insulin at all. The body's own immune system, the system that's supposed to fight off infections, mistakenly attacks and destroys the specific cells in the pancreas that produce insulin. Nobody fully knows why it turns on those cells, but once they're gone, they're gone, and insulin production stops.

Because no insulin is produced, people with type 1 take insulin from outside the body for life.

And you already know what insulin does, so you can see the consequence right away. With no insulin, there's nothing to tell the cells to open up. Glucose has no way in. It piles up in the blood while the cells themselves go hungry. This is why people with type 1 have to take insulin from outside the body, usually by injection or a pump, for the rest of their lives. They're replacing a signal their body can no longer send.

Type 1 often shows up in childhood or young adulthood, and it isn't caused by anything a person ate or did. It's roughly 5 to 10 percent of all diabetes.

What happens in type 2 diabetes

Type 2 is the far more common kind, around 90 to 95 percent of cases, and it's probably why most people are reading this. It works differently, and the difference matters.

In type 2, the pancreas can still make insulin, at least at first. The problem is on the other end. The cells stop responding to insulin the way they should. Insulin knocks, delivers its message, and the cells more or less ignore it. This is called insulin resistance, and "resistance" is a good word for it: the cells are resisting a signal they used to obey.

Because stage 1 is silent, type 2 often goes unnoticed for years before it's caught.

Now, your pancreas doesn't give up easily. When it notices blood sugar staying high, it assumes there isn't enough insulin, so it makes more. And for a while, this actually works. Extra insulin is enough to force the stubborn cells to take in glucose, and blood sugar stays roughly normal. This can go on quietly for years, which is exactly why type 2 often goes unnoticed for so long. Behind the scenes, though, the pancreas is working overtime.

The trouble is that it can't keep up that pace forever. Over time, the pancreas starts to wear down and can no longer produce enough insulin to overcome the resistance. Now you've got both problems at once: cells that don't respond well, and not enough insulin to push them. Glucose builds up in the blood, and blood sugar readings climb into diabetes range. That's usually the point where it gets caught.

The good news buried in here is that insulin resistance can often be improved, and we'll come back to that, because it's the reason the daily habits you've been told to adopt actually work.

Why diabetes makes you feel the way it does

This part tends to be the moment things click for people, because suddenly the symptoms aren't random. Every one of them traces straight back to sugar being stuck in the blood.

Once you see that the symptoms are just the visible edge of the same underlying thing, the whole condition feels less like a random collection of problems and more like one story you can actually follow.

Why it happens to some people and not others

It's natural to wonder why this happened to you, and it's worth saying plainly: a diagnosis is not a verdict on your character or a punishment for past choices. Type 2 diabetes comes from a mix of things, and most of them aren't fully in anyone's control.

Genetics play a real role, which is why diabetes tends to run in families. Age matters too, since insulin resistance becomes more common as we get older. Carrying extra weight, especially around the middle, and long stretches of not moving much both make cells more resistant to insulin. And plenty of factors sit outside the individual entirely, like what food is affordable and available where you live, how much time and safe space you have to be active, and stress that never quite lets up.

In other words, this landed on you through a tangle of biology and circumstance, not because of some failing. That matters, because shame doesn't help anyone manage a condition. Understanding does.

Why understanding this makes the advice make sense

Here's the payoff for reading all of this. Once you know the mechanism, the standard advice stops sounding like arbitrary rules and starts sounding like what it is: direct ways to fix the actual problem.

Think about why a short walk after eating helps. When you use your muscles, they can pull glucose out of your blood for fuel with much less insulin than usual, almost as if activity temporarily turns down the resistance. A ten-minute walk after your biggest meal isn't busywork. It's giving that mealtime glucose somewhere to go.

The same logic explains the food advice. When you eat a plateful of fast-digesting carbohydrates, a big wave of glucose hits your blood all at once, and an already-struggling system has to deal with the surge. Balancing that plate with vegetables, protein, and some fat means the glucose arrives more gently and gradually, so there's less of a spike for your insulin to chase. You're not being punished with "bad" foods and "good" foods. You're just smoothing out the load on a system that's having a hard time keeping up.

None of this means the condition is simple to live with, and none of it replaces the plan you and your doctor work out together. But when you understand what's happening inside you, you stop following instructions blindly and start making choices that you actually understand the reasons for. That shift, from being managed to understanding your own body, is where a lot of people find their footing.
